• Eligibility is extended to the employee and their spouse, parents, grandparents & children
• Location: Any of the 4 Hearing Health Center locations in Chicago & Suburbs
• Hours: Call one of our offices Monday through Friday
• Must provide current picture ID to take advantage of benefits
